I used to work somewhere, that should have had that scale of accident!!!!

when I worked in the paint factory in Morley the boss wanted the forklift guy to start loading the truck. He refused as it was break so the boss got in the fork lift and went to lift a pallet tower to start loading and put the forks all the way though and into the pallets beyond knocked down 12 pallets of paint the truck didn't leave that day.
