Boel iTap exploded a bottle

Hello, I have a Boel iTap and was trying to fill with it a couple of days ago. I was purging a Tynt Meadow bottle and it exploded, sending a shard of glass flying out narrowly missing my face. As you can imagine it was a bit of a shock. Is this some freak accident or is it possible to put the pressure too high? I think it was set to ~18PSI.
 
I've regularly used a different counterflow filler at 12 psi with no exploding bottles so far including some Tent Meadow bottles. Cant imagine an additional 6psi is going to make a huge difference. Maybe it was cracked or something?

Yeah it was probably a bad bottle. I had a beer tasting with some friends which is what the bottle came from, I wouldn't be surprised if it got dropped or thrown into the recycling which damaged it.
