I have made 4 all grain brews so far and I reckon I have 65% efficiency.
The way I have worked it out is by making a recipe using 70% efficiency values and altering the efficiency percentage in the brewr software I use until I get an SG that matches what I had.

I would disregard the aroma steep and whack 40g in at five minutes. You will get the benefit of the steep as you cool the wort. It will also increase your overall IBU slightly.

In my opinion, if you want hops, leave the steeping grains out and go only with the base malt.
I would also combine your Amarillo additions and put them in at five minutes leaving them as the wort cools. Adjust the 45 minute hops to provide the Bittering you want or boil for an hour.

In response to question 1 I would suggest that you download brunwater and have a look at that with your water figures.
