You seem to have two ball lock disconnects

I'm not convinced that black ball lock disconnect is seated fully.

The QD is not seated right or is bad. The o-ring is missing or has rolled off of the post seat. Also be sure to use a little keg lube on the o-ring.

I think the "o" ring isn't seating, that screw on the top of the disconnect, have you got it tightened down?
